# Cake order form template

Four questions do most of the work: allergies as their own question, the date checked against your calendar, the exact spelling of any writing, and budget asked in bands.

## The form

**About you.** Your name (required). Phone number (required). Email (required). How did you hear about me? (optional)

**The occasion.** What date do you need it? (date, required). Pickup or delivery? (required; ask the address only in the delivery branch, and state your area and fee). What time? (your own windows, required). What is the occasion? (optional)

**The cake.** How many people does it need to serve? (number, required). Flavour (from your list, required). Filling (from your list, required). Does anything need writing on it? (Yes/No, required). If yes, type it exactly as you want it to appear (shown only on Yes). Photos of what you have in mind (upload, optional, allow several). Anything else I should know? (long answer, optional)

**Allergies.** Does anyone eating this have a food allergy or intolerance? (Yes/No, required). If yes, please list them (long answer, required when shown).

Put your kitchen statement directly under these two: "My kitchen is not allergen free. I bake with nuts, wheat, dairy and eggs, and I cannot guarantee against traces of any of them."

**Budget and confirmation.** Roughly what budget did you have in mind? (bands, optional). I understand my date is not held until the deposit is paid (checkbox, required).

## Why these

**Allergies get their own question.** Folded into a general notes box it gets skipped. Asking yes or no first and only then asking what forces an answer, because a required question she must actively answer "no" to is far harder to scroll past than an empty optional box. Put the kitchen statement beside it, not in your bio: the person reading the allergy question is exactly who needs it.

**The date box is only useful if it knows your calendar.** A plain date field will accept a Saturday you are already full on. Block dates and set a lead time if your tool allows it. If not, say it above the box: "Custom orders need at least two weeks. If your date is sooner, message me before filling this in."

**Ask for the exact spelling, in its own box.** "Mackenzie" is four names depending on who spells it, and piped icing is not fixable at four in the afternoon. Copy and paste what she typed rather than retyping it.

**Budget in bands, lowest band at your real minimum.** A blank box makes people guess low or leave it empty. Bands mean the person wanting a three-tier wedding cake for $80 finds out now, from a list, rather than from you later.

**The checkbox is the cheapest thing here.** It sets the expectation before she is emotionally committed, and it records that she actively agreed, which is worth much more than fine print she never scrolled to.

## What to leave off

Tiers and inches (ask servings). Any question whose answer you have never used. An address on a pickup order. Anything you could answer yourself, like your flavour list.
